Summary

Regeneron's Q3 2026 Sanofi profit share rose 77% to $2.575 billion after the development balance was repaid, and operating income nearly doubled to $2.048 billion.

Regeneron (REGN) reported third-quarter 2026 results on October 30, 2026. Revenue rose 36% to $5.116 billion and GAAP operating income rose 99% to $2.048 billion [1][2]. The Q3 2026 earnings answered three open questions positively: whether Dupixent profit sharing would turn fully into revenue once the Sanofi development balance was repaid, whether EYLEA HD could absorb losses from the older EYLEA, and whether higher spending would leave the profit gain intact. EYLEA HD conversion ran ahead of the company's demand guidance, and operating income was $755 million higher than the second quarter's $1.294 billion [3][4]. The caveats are R&D above original guidance, falling ex-U.S. EYLEA profit share and weaker cash flow.

Regeneron discovers, manufactures and sells antibody medicines, and it sells its products mainly to wholesalers and specialty distributors [5]. The company sells the eye drugs EYLEA HD and EYLEA in the U.S. and Libtayo globally, shares global Dupixent profits with Sanofi, and splits ex-U.S. EYLEA profits equally with Bayer [5]. Sanofi collaboration revenue made up 53% of total revenue this quarter, its largest revenue source [2].

Dupixent profit share turned fully into revenue

Regeneron's share of Sanofi collaboration profits was $2.575 billion in the third quarter, up 77% year over year [2]. This was the first full quarter after the development balance (Regeneron's obligation to repay development costs Sanofi funded earlier) was fully paid off, and the deduction this quarter was zero [6]. The second-quarter recognized amount was $2.033 billion, or $2.286 billion before the deduction, so the third quarter was 12.7% above even the pre-deduction level [7]. About $250 million of the increase came from the deduction ending; the rest came from higher collaboration profit.

Dupixent sales drove the growth in collaboration profit. Dupixent global net sales were $6.541 billion in the third quarter, up 35%, including $5.064 billion in the U.S., the first quarter above $5 billion there [8][9]. Regeneron's profit share as a percentage of combined Dupixent and Kevzara sales rose to 38% [10]. The higher ratio fits a larger U.S. sales mix, but Sanofi's shared expenses are undisclosed, so the causes cannot be fully separated. This revenue carries no matching cost of goods sold.

EYLEA HD absorbed U.S. losses

EYLEA HD U.S. net sales were $699 million in the third quarter, up 62% year over year and 17% from the prior quarter [3][11]. U.S. sales of the older EYLEA were $400 million, down 41% year over year but only 3% below the second quarter [11][12]. The combined U.S. franchise reached $1.099 billion, up 9% sequentially, and EYLEA HD's share rose to 63.6% [11]. The company attributed EYLEA HD growth to higher volumes driven by demand, partly offset by a lower net selling price [3].

Management said more biosimilars are about to launch and pricing pressure may offset part of the demand growth [13]. The EYLEA HD pre-filled syringe has not yet been approved, and the company is targeting approval by the end of 2026 [14]. Outside the U.S., Regeneron's share of Bayer profits was $212 million, down 32% year over year, so erosion abroad is continuing [15].

Profit was retained as R&D accelerated

Operating income kept the collaboration profit gain in the third quarter, and gross margin on net product sales recovered to 82% with no manufacturing-interruption costs this quarter [1][16]. R&D expense was $1.795 billion, up 22%, above the roughly $1.73 billion per quarter implied by the original full-year guidance [17][18]. SG&A expense was $678 million, below the range implied by the original guidance [17]. The two combined remained within the original guidance range.

The company adjusted its full-year guidance accordingly. It raised GAAP R&D guidance by $100 million to $6.600-$6.735 billion, cut SG&A guidance by $100 million and raised the gross margin outlook to about 80% [19]. Management expects 2027 R&D to grow by a mid-teens percentage, and third-quarter other revenue included a one-time $60 million cumulative catch-up adjustment [20].

Conclusion

The quarter strengthened the view of Regeneron's growth and profit conversion: Dupixent profit share kept growing without the deduction, EYLEA HD stopped the decline in U.S. eye-care revenue, and operating income rose sharply even as R&D stepped up. Cash generation has not kept pace. Free cash flow for the first nine months was $2.904 billion, below $3.159 billion a year earlier, and accounts receivable rose by $1.84 billion [1]. Regeneron will recognize a one-time $1.0 billion Sanofi up-front payment in the fourth quarter. The next tests are whether profit share holds flat excluding that payment, whether EYLEA HD keeps growing as new biosimilars launch, and whether faster R&D begins to erode the profit gain [2].
